Student COVID Forms
The District is moving from a weekly lab testing program to a weekly over-the-counter (OTC) testing program. The District will supply the OTC test kit each week to registered students. Tests must be administered at home. Submission of a weekly online attestation form is required no later than Friday at 1:00 PM in order to receive an OTC test kit the following week. Students only have to register once and will remain in the program so long as an attestation form is completed on a weekly basis. Failure to complete a weekly attestation form will remove the student from the program. Students may register again and can rejoin the program at the beginning of the following month.
